It is our desire to make the conference affordable to as many local as possible. However many of the nurses in rural Jamaica and students may not be able to afford the cost of registration, transportation and accommodation. To contribute to the registration cost of local nurses or students please include the amount as part of your registration payment.

OSDJ is a 501(c) (3) corporation registered in the State of Minnesota. Contributions made to provide scholarships to local students and nurses to attend the conference are tax-deductible.  (OSDJ Tax ID: 36-4496126)

Registration Deadlines
1. The deadline for early registration is December 15, 2008.
2. Late registration closes February 15, 2009

ON-SITE REGISTRATION
On-site late registration will be available 8:00 a.m. through 3:00 p.m. at the external sessions and conference site. On-site registration must be accompanied with payment by a major credit card, check or cash. This is a LIMITED registration at a greatly increased rate and OSDJ does not guarantee that tickets to events, materials, welcome bags or other opportunities will be available to those who do NOT register by January 31, 2009.

REGISTRATION CONFIRMATION
Upon receipt of your registration payment, OSDJ will e-mail you a registration confirmation. All subsequent updates will be sent via email. Please keep your email address current with OSDJ.

Hotel and Ground Transportation

**Rates are All-Inclusive per room per night

Online Hotel Reservations must be made by January 20, 2009 in order to get the above rates

Double Occupancy   - two (2) adults and two (2) children under 12 years

Triple Occupancy    - three (3) adults maximum (2 double beds)

The hotel will make arrangements for triple rooms with 2 children if adults and children are part of the same family and they don't mind sharing the room.

 

Rates apply to extended days around the conference. Please indicate actual dates if different from Conference Days

CANCELLATION POLICY

Registrants who are unable to attend the conference will receive a refund less a $60 administrative fee if a written cancellation is received by December 15, 2008. Cancellations must be emailed to osdj@aol.com No refunds will be granted after December 15, 2008. OSDJ reserves the right to cancel any program and assumes no responsibility for personal expense.
